These are referral resources available for diabetic patients who get care in Pittsburg or Baypoint

  • Promotoras: They offer culturally-sensitive nutrition and diabetes classes, a peer health support group (Alma con Alma), patient navigation (i.e. helping get insurance, appointments), and, in 2010, Zumba classes

    • For Baypoint, contact Elizabeth Luciano at 427-8338
    • Alma con Alma is every Friday, classroom B, with free snacks and free child care. Patients can just drop in. No health insurance required. "Pre"-diabetics are welcome, too.
  • Dietician-run diabetes classes (Rosa Venegas and Sandy Quierolo): two-part English- and Spanish-language classes
    • Use MR191, and nurse can schedule via EPIC
    • English: Part I is 1st and 3rd Wed at 2 PM, part II is 2nd and 4th Wed at 2 PM
    • Spanish is 1st and 3rd Thur at 2:30 PM, part II is 2nd and 4th Thu at 2:30
  • African-American Health Conductors: They offer a peer health support group (called "Body and Soul" in Pittburg and "Soul to Soul" in Baypoint) and patient navigation

  • Diabetes class of Dr. Desoer (8-part class, 3rd Thursday morning of each month, from 9 - 11)
    • Contact Jessica Jones at 431-2398
    • Next class, starting March 18, will be in English
    • Please do NOT schedule patients into this class directly into EPIC yourself
  • Patient educator (e.g. for teaching how to administer insulin)
    • Use MR191, and nurse can schedule in EPIC,
    • Patients can also call with questions (or to cancel appointments), at 431-2332
